Are laboratory workup really needed on management of pediatric seizures at emergency rooms?

Abstract
This study was carried out to determine the value of workup done at emergency departments (EDs) on the management of pediatric seizures. We retrospectively reviewed all patients with seizures admitted to Ege University's Childrens Hospital's ED between March and December 2008. All demographic findings, seizure characteristics and laboratory workup were recorded. Eighty-three children were eligible. The mean age was 5.5±4.6 years; majority seizures (83%) lasted less than 10 minutes of which 66% were afebrile. Although, most patients had blood workup (92%), cranial computerized tomography scan or electroencephalogram was carried out in only five patients. Metabolic abnormalities were detected in 25 patients. The frequency of metabolic abnormalities was significantly higher in patients under two years of age (53.4% vs 24.4%) (p=0.02). No seizures occurred due to metabolic abnormalities. CT scans and EEG were abnormal in a patient with tuberosclerosis. Six patients were hospitalized. Altghough, blood workup, CT scans and EEGs are frequently obtained in patients with seizures at EDs, they have a limited value on management. Our results showed that extensive workup may be helpful in selected patients, i.e those younger than two years of age, that have either focal seizures or seizures lasting longer than 10 minutes.

Role of catecholamine in tumor angiogenesis linked to capacitance relaxation phenomenon

Abstract
The present paper describes the role of catecholamine with chromogranin A(CgA) in tumor angiogenesis linked to capacitance relaxation phenomenonl. The capacitance relaxation phenomenon is the observation about the capacitance value which jumps rapidly in cancer cell around 200Hz.It has been established that catecholamine is of recent interest in regulation of angiogenesis. The present paper deals with chromogranin A (CgA), coreleased with catecholamine in plasma in patients suffering from pheochromocytoma(PHEO), multiple endocrine neoplasia (MEN), medullary thyroid carcinoma (MTC), and congenital adrenal hyperplasia(CAH) and patients with others endocrine disorders. The paper also discusses the progressive rise in CgA with catecholamine linked to capacitance relaxation phenomenon. The experimental results of capacitance relaxation phenomenon were given as inputs to a model for correlation with the CgA level and catecholamine. It is to be noted that the model is simulated in MATLAB 7.0. MATLAB allows matrix manipulations, plotting of functions and data, implementation of algorithms, creation of user interface and interfacing with programs written in other languages.

Does the ESR-adjusted serum ferritin concentration predict iron deficiency better?

Abstract
There has always been a need to develop simple and accurate techniques to improve the predictive value of the serum ferritin concentration (sFC) in patients with systemic inflammation and malignancies. This prospective study examined the impact of adjusting sFC for acute phase reactants with the erythrocyte sedimentation rate (ESR), using the bone marrow aspirate Perl’s stain as a gold standard for comparison. Sixty patients (male/female = 35/25), with a mean age of 44 ± 4 years were recruited. Thirty-four of the patients had HIV infection, twenty had haematologic malignancies, four had unexplained severe anaemia, while two had hyperreactive malarial splenomegaly. Haematologic parameters including ESR, ELISA-based serum ferritin concentration (sFC) and bone marrow aspiration (and trephine biopsy, if aspirate was aparticulate) were examined. The effect of ESR-adjustment of the sFC on the iron status predictive value was studied. The difference in the sFC of bone marrow aspirate (BMA)-negative and BMA-positive patients was significant, and was further enhanced with ESR adjustment of the sFC. This study has shown clearly that the predictive value of sFC for assessing iron status can be enhanced using the ESR, in patients with chronic inflammatory conditions and malignancies.

Early preventive measures are effective against transmission of rotavirus disease from person to person in the hospital environment.

Abstract
An observational surveillance study was carried out to describe the outbreak of rotavirus in a pediatric hospital in Mexico City. A total of 13 cases of intra hospital rotavirus infections were identified. None of these cases was vaccinated against rotavirus. The average duration of hospitalization of the patients was 11 days (range 3 to 33 days). Implementation of contact precautionary measures successfully ended the outbreak 21 days after detection. All cases were successfully managed and treated with symptomatic measures. We corroborate the importance of implementation of early preventive measures in intra hospital outbreaks of rotavirus for adequate control.

Nalidixic acid resistance and clonal expansion of SALMONELLA ENTERICA serotype Paratyphi A Isolates in Yuxi city, China

Abstract
Antimicrobial susceptibility tests were performed with 4060 strains of SALMONELLA ENTERICA serotype Paratyphi A (SPA) isolated from patients at Yuxi City from 1999 to 2008. The incidence of resistance to nalidixic acid of the isolates increased from 12.5% in 1999 to 82.2% in 2000, 93.0% in 2001, and 100% in 2008. The frequencies of intermediary and susceptibility (Kirby-Bauer disc diffusion technique) to ciprofloxacin were 17.0% and 83.0% before 2005, 65.3% and 34.7% in 2008, respectively. A group of 166 NAR and 20 NAS isolates of SPA were typed by pulsed-field gel electrophoresis (PFGE) using SpeI into nine different PFGE patterns with predominance of the SpeI01 and SpeI02. All NAR isolates had higher MICs for fluoroquinolone. We validate the use of the nalidixic acid screening test for detection of decreased fluoroquinolone susceptibility in SPA.

Cataract blindness in a Nigerian tertiary hospital- A one year review

Abstract
Blindness negatively affects quality of life and leads to increased mortality. Available community based studies showed that three quarters of the causes of blindness majorly by cataract, is avoidable. This study was conducted so as to have hospital based data on cataract as a cause of blindness in University of Port Harcourt Teaching Hospital, (UPTH), Nigeria. The case records of all blind patients (visual acuity of less than 3/60 or corresponding visual field of less than 100 in the better eye with the best possible correction) presenting at the out patient clinic for year 2008 were reviewed. Of 214 patients, Cataract was responsible for about one third (n=82.38.3%). The age 40-60 years had the highest proportion of bilaterally cataract blind patients (40%). More males were bilaterally cataract blind (n=12.60%). Most bilaterally cataract blind patients lived in the rural areas (n=12.60%). The uptake of surgery was poorer among bilaterally cataract blind patients with only 30% (n=6) presenting for surgery. Cataract is a leading cause of blindness in the UPTH, Nigeria and is commoner in the older age group especially among males. The rural dwellers present late especially when bilaterally blind from cataract. Subsidizing the costs may improve the uptake of cataract surgery in our center.
