Prevalence of priapism in cases suffering from sickle cell disease in Madhya Pradesh of Central India

Abstract

The sickle cell disorder is a common disabling disease that profoundly affects human morbidity, mortality and quality of life. Priapism in sickle cell disease is a condition that causes sustained, painful, and unwanted erection of the penis in boys or men without any sexual stimulation. Sickled cells are short lived and can cause vaso-occlusive crisis in affected persons. A few case reports have shown the rare occurrence of priapism in sickle cell disease in India. This hospital based study showed around 2.97% prevalence of priapism in cases suffering from sickle cell disease in a tertiary hospital at Jabalpur in Madhya Pradesh of Central India. The present study reports the lower fetal hemoglobin (mean, 8.5%) in two of the three cases, and elevated serum bilirubin level (mean, 4.4 mg/dl) and higher platelet (mean, 351.7 m/mm3) and reticulocyte (mean, 8.7%) counts of all patients studied with priapism in sickle cell disease, indicating erythropoietic imbalance. It is a common practice in India that any problem related to sex or sex organs is intentionally not disclosed (kept secret) to elderly family members because of shyness or psycho-social implications or due to a stigma attached to it. It is rarely brought to the notice of a medical doctor. This is due to utter lack of sex education that results in deteriorated health complications in rural areas of the state. Priapism in sickle cell disease is a pathological condition of penile erection and recovery of erectile function is dependent on prompt counseling and urgent intervention. Priapism should be considered as a medical and surgical emergency and efforts should be made to educate both professionals and vulnerable population as well.

Abstract

This is a case report of 30 years old female patient who presented with almost monthly episodes of urinary symptoms misdiagnosed and treated as recurrent urinary tract infections with poor response. On physical examination of this lady, features suggestive of lichen planus involving both the oral and genital area were found and immediately she was placed on systemic and local immunosuppressive therapy with excellent response for almost a year, but unfortunately she was lost for follow up.

Attitudinal index influencing the trend of child health care services in Cross Rivers State, Nigeria

Abstract

This study investigated the relationship between attitude indices and utilization of maternal/child health care services among women in Akpabuyo Local Government Area (LGA) of Cross River State, Nigeria. The study isolated and examined mothers’ level of education, distance to health care facilities, attitude of health care givers, cultural beliefs, and the utilization of maternal health care services. A validated and reliability certified structured questionnaire was used to generate data from 150 subjects among nursing and pregnant mothers using the convenience sampling technique. Data generated were analyzed using the Pearson Product Moment Correlation statistics. The result of the analysis showed that level of education of mothers, distance to health facilities, attitude of health care givers, and cultural beliefs were significantly related with utilization of maternal/child health care services in Akpabuyo Local Government Area of Cross River State. Based on the result of the study, the following recommendations among others were proffered: Advocacy and practical approach to the girl-child education should be pursued by the government at all levels; and that adult literacy education should be instituted in Akpabuyo Local Government Area, with maternal and child health as core component of the programme.

Quality of life after septal surgery

Abstract

To assess the quality of life in patients who have been through the surgical intervention related to nasal septum in ENT department of Abbasi Shaheed Hospital, Karachi. This study was done from January 2008 to August 2008. Case based study, with post-interventional quality of life assessment questionnaire. One hundred patients undergoing nasal septum related surgery, after their full informed and written consent, were included in this study. The age limit was above 18 years and both the sexes were included. They were given a questionnaire (Glasgow benefit inventory score-GBI) to be fulfilled on the first follow up day after a week’s time from the day of surgery and then subsequently on the second visit that is, at about two week’s time from their first visit and the last one after the lapse of 1 month from the time of second visit. Average score of patients as per the GBI on the first follow up visit was 17; on second subsequent visit was 39 and on the last visit it was 50 thereby showing a positive correlation between the improvement in quality of life and septal surgery. Septal surgery has a definite and positive impact on improving the health related quality of life of patients.

Circumcision mishaps: Sequelae from the hands of the untrained personnels

Abstract

Male circumcision is one of the oldest surgical operations documented on earth. It is performed both for religious and cultural reasons. It is being carried out by both trained and untrained persons leading to several complications. The psycho-social effects are overwhelming to both the victims and their relations. Reports of adult cases are very rare because circumcision is commonly carried out in neonates. The article attends to illustrate that untold complications from circumcision by untrained personnel could occur and present in adulthood or uncommon age groups.  A series of three case, two adults and one infant, were reviewed for rare complications of amputation which were managed in Bayelsa State. These circumcision cases illustrated that some of the long term complications arisen from circumcision by untrained personnel. Hemorrhage, infections, penile necrosis, glans amputation, and buried penis were the complications reported in the literature. To avoid these complications, trained personnel must carry out the Circumcision.

Gastro-intestinal stromal tumours of stomach and small intestine: Report of two cases

Abstract

Gastrointestinal stromal tumors (GIST) are rare gastrointestinal tract (GIT) tumors that originate from the interstitial cells of Cajal. Melena or hematemesis and anemia are the most common presentations and CT scan is the investigation of choice in demonstrating the endoluminal and exophytic extent of tumor. Surgery remains the mainstay of treatment in GIST and chemoradiation has not proved to be very effective. We have encountered two such cases of GISTs in our clinical practice. 62-years and 32-years old males presented with epigastric and paraumblical pain respectively. Clinical examination revealed a well defined epigastric mass of approximate size 12*10 cms in former and a vague paraumblical mass in the later. Contrast Enhanced CT Scan of abdomen and pelvis suggested the origin of masses in each case and exploratory laparotomies performed in both of them revealed a mass in relation to the stomach and ileum respectively. Histopathology and immunohistochemistry confirmed the diagnosis. Histopathology and immunohistochemistry are very useful aids in making a diagnosis of GIST and surgery remains the choice of treatment even in cases of larger GISTs.
