Severe caries: A clinical dilemma

Abstract

The microbial etiology of dental caries is discussed in terms of the dynamic relationship among the dental plaque microbiota, dietary carbohydrate, saliva, pH lowering and the cariogenic potential of dental plaque. Herein, a case is presented in which the main cause of the caries for this pampered patient was compromised oral hygiene maintenance, after a history of a severe accident, in which he lost his digits. This case report emphasizes the proper evaluation of history, which leads to proper diagnosis and treatment planning. Educating people about the etiopathology and introduction of preventive and maintenance strategies does not only assists in meeting the special oral needs of the adolescent population, but also helps to establish lifelong healthy habits.

A case of recalcitrant oral lichen planus

Abstract

Oral lichen planus (OLP) is a chronic inflammatory dermatosis of unknown etiology that often involves the mucous membranes. Most of the non-ulcerative type of OLP improved with topical and systemic medications and recurrence is common. Here, a 52-year-old male presented with a 10-year history of persistent, gray-white oral mucosal discoloration and discomfort that had not improved after empiric treatment with topical triamcinolone acetonide. Histopathologic examination confirmed the diagnosis of OLP. Treatments were given with intralesional triamcinolone acetonide, oral paste and even systemic steroids, but little improvement was noted. Finally, patient was cured by surgical excision. There was no relapse after 2 years follow up.

A case of an unusually large Sublingual dermoid cyst of the maxillofacial region

Abstract

Dermoid cysts are true developmental cysts, which arise from the entrapment of pluripotent ectodermal and mesodermal primordia in the embryonic lines of fusion, between the 3rd and 5th weeks of gestation. Hence, they are dysontogenic in origin, that is, they result from defective embryonic development. In the head and neck region, these ectodermal and mesodermal components get entrapped when the 1st and 2nd branchial arches of each side fuse in the midline. The entrapped tissues then undergo proliferation and cystic transformation. A case of a large sublingual dermoid cyst in a 37 year old male patient is presented which closely mimicked a plunging ranula and was successfully managed by surgical excision.

Long-term changes in graft height after sinus floor augmentation with mesenchymal stem cells in a randomised clinical trial

Abstract

To compare the changes in graft height in patients after sinus floor augmentation (SFA) with and without mesenchymal stem cells (MSCs) and to evaluate the survival rate of implants placed after SFA. Material and methods: Twenty patients were randomised into two groups. The test group patients had a bone biopsy taken from the tuberosity region from which bone cells were cultured. All patients had a SFA with a composite bone graft (BioOss®/autogenous bone). The patients in the test group had the cultured cells added. Two implants were placed in the augmented area four months later and the augmentation height at the distal side of the implants was evaluated from panoramic radiographs at the time of implant placement (T1) and after a minimum of 2.5 years (T2). The implant survival was evaluated. Due to insufficient bone quality one implant could not be placed 4 months after SFA, two implants disintegrated after prosthetic treatment and two implants in one patient were unaccounted for since the patient failed to show up – all in the MSC group. A statistically significant reduction in augmentation height of 1.27 ± 0.23 mm and 1.88 ± 0.37 mm in the MSC and non-MSC group was found after a minimum of 2.5 years, respectively (p<0.01). No overall difference between the two groups was found (p=0.18), however significantly less reduction in augmentation height was found in the most anterior implant position in the MSC group compared to the non-MSC group (p<0.05). Adding MSCs to SFA did not improve implant survival after a minimum of 2.5 years. A reduction in augmentation height of 10-13 % was found. The anterior augmentation height was more stabile when MSCs were added to the bone graft.

Risks of infection on odontological procedures

Abstract

This study assesses the routine adopted by dentists in Alfenas, Minas Gerais, during clinic practices, regarding to biosecurity procedures usage. In this study, 41 participants were randomly selected out of a roll provided by the Odontology Regional Council in Minas Gerais (CRO-MG), and were divided into three groups considering gender, general or specialized activity and period of college graduation. Misconceptions were found regarding to hand hygiene, clinical procedures done with bare hands and absence of protocol that should be adopted in case of accidents with sharpened objects. The results show the performances done with lack of information or standardization that improve the adoption of conduct that validates the procedures for biological protection.

Endogenous and exogenous teeth whitening through gel-based bleaching hydrogen peroxide (35%) with the aid of silicon guide

Abstract

The aim of this study is to demonstrate through a case report, a proposed treatment for discolored teeth, with and without pulp vitality, by the technique of external and internal tooth bleaching with hydrogen peroxide to 35% Lase Peroxide Sensy (DMC) using Whitening Lase II Device (DMC), and a silicone guide (3M ESPE) in the palatine portion of the upper teeth. In this clinical case, the patient had darkened dental elements 11 and 22, and dissatisfaction with the coloring of other elements. It was observed that the techniques used and the materials chosen allowed for an excellent aesthetic result, with technical simplicity and low cost, and minimal occurrence of signs and symptoms.
