Why do patients prefer Khyber College of Dentistry for dental treatment? A hospital based study

Abstract
The objective of this study was to investigate and analyze the most common reasons for patients visiting Khyber College of Dentistry, Peshawar Khyber Pakhtunkhwa, for dental treatment rather than visiting other dental care centers and private clinics. About 300 patients who visited Khyber College of Dentistry during the period from 20th October 2010 to 5th November 2010, irrespective of gender, were interviewed using a specially designed questionnaire. They were asked to give feedback regarding different reasons for undergoing treatment at this hospital. Males (40%) were remarkably dominated by females (60%) in this study, with 1:1.5 ratios. Patients in their 3rd decade (41.43%) were frequently interviewed followed by 4th decade (20%). Majority of these patients were illiterate. Most of the patients responded that they visited this hospital because of feasible access. 37.14% were referred by their relatives and Doctors (n=40, 28.57%). In response to a question regarding past experiences, 52.86% patients had a satisfactory experience with this hospital. Khyber College of Dentistry is attracting more patients in terms of quality and care. The future of this institution will be built on these bases to improve the delivery of dental care. Patients are directly accessing information on new technologies and treatment methods.

Effects of different concentrations of bleaching agent on the micro hardness and shear bond strength of restorative materials – An in vitro study

Abstract
With the growing awareness of esthetics the use of bleaching agents are on the increase. Bleaching agents intended to be used at home affects the existing restorations. The aim of the present study was to evaluate the effect of bleaching with carbamide peroxide agents at concentrations 10 and 22% on the microhardness and shear bond strength of composites and compomers. For microhardness and shear bond strength evaluation, samples of both composite and compomer restorative materials were subdivided into control group (stored in artificial saliva), Experimental groups bleached with 10% CP and those bleached with 22% carbamide peroxide. Following 2 weeks of bleaching treatment, Vickers’s hardness number was noted for each test specimen using a microhardness meter. For shear bond strength evaluation, the specimens were subjected to shear test under Instron universal testing machine. One way Analysis of Variance (ANOVA) was used for multiple group comparisons followed by Tukey’s test for pair wise comparison. Bleaching with carbamide ceroxide at concentrations 10 and 22% caused reduction in hardness of composite restorative materials. However bond strength of both composite and a compomer restorative material was not affected following bleaching treatment. Hence it can be deduced that bleaching caused reduction in hardness of composite restorative materials.

Remineralization potential of 5,000 ppm fluoride dentifrices evaluated in a pH cycling model

Abstract
Prescription 1.1% sodium fluoride (NaF) dentifrices designed to either have fast dispersion for improved enamel fluoride uptake (that is, PreviDent® Booster 5000) or contain an innovative tricalcium phosphate system for enhanced remineralization (that is, Clinpro® 5000) were evaluated for anticaries potential in an in vitro pH cycling model. Polished bovine enamel specimens were initially softened in a white- spot-forming solution comprising 0.1 M lactic acid plus 100 kDa polyacrylic acid (PAA, pH = 5.0) for 26 h at 37°C. Specimens were then measured for baseline Vickers microhardness and stratified (N =into the following groups: Group A: Tom’s of Maine fluoride-free dentifrice (negative control); Group B: Colgate PreviDent® Booster 5000 (5000 ppm fluoride) and Group C: 3M Clinpro® 5000 (5000 ppm fluoride). The groups were then cycled for 10 days in a pH cycling model consisting of four one-minute treatment periods (diluted 1:3 with distilled water) and one four-hour acid challenge (lactic acid-PAA , pH = 5.0) per day. Between these events, specimens were immersed in artificial saliva (pH = 7.0). After 10 days of cycling, the specimens were evaluated for Vickers surface microhardness, mineral loss and lesion depth using microindentation, transverse microradiography and polarized light microscopy. For all analyses, statistical differences (t-tests, p<0.05) were found to exist among the groups, with Clinpro® 5000 conferring superior surface and subsurface remineralization potential relative to both PreviDent® Booster 5000 and Tom’s of Maine fluoride-free paste. Due to this superiority, these results suggest the combination of 5,000 ppm fluoride plus the tricalcium phosphate system may provide significant anticaries benefits relative to fluoride-only and fluoride-free dentifrices.

Flare-up incidence and related factors in adults

Abstract
The study determined the incidence of flare-up and the effect of visit type, treatment duration, pre-operative pain, etc. on flare-up in adults. One hundred and seventy five participants, aged 18 to 60 years with a necrotic central incisor, with or without pre operative pain, participated in the study. They received post operative paracetamol tablets and were asked to report back if unbearable pain/swelling developed. A 10% flare up rate was recorded, while none of the studied factors had a significant relationship with flare-up, except intra operative pain.

Types of removable prostheses requested by patients who were presented to the University of Benin Teaching Hospital Dental Clinic

Abstract
To determine the most frequent type of removable prostheses requested by patients who presented to the University of Benin Teaching Hospital outpatient clinic a descriptive retrospective study was carried out for all patients attending the Prosthodontic out patient clinic, who requested for removable prostheses from January 2004 - October 2008. The source of data was the clinic’s log book and patients’ case notes. Exclusion criteria are; request for obturators, mouth guard and palatal feeding plates for cleft lip and palate patients. Data of interest were: sex, age, edentulous space to be restored and reason for restoration. Data was analyzed using statistical package for social sciences version 13.0 and presented as a simple frequency table. A total of 351 cases were seen during the study period. This was made up of 188 (53.6%) males and 163 (46.4%) females. The age ranges were < 20 years (1.4%), 21 - 40 years (68.9%), 41 - 60years (19.9%) and 61 - 80years (9.8%). Edentulous spaces restored in these patients were Kennedy’s class III (57.3%), Kennedy’s class IV (26.2%), Kennedy’s class I and II (0.9%), Kennedy’s class III with modifications (5.7%), Kennedy’s class II with modifications (1.4%), Kennedy’s class I with modifications (1.7%) and complete dentures (6.0%). The main reason for seeking replacement was aesthetics (89.2%). The most common removable prostheses requested by patients who attend the University of Benin outpatient clinic was Kennedy’s class III, with aesthetics being the main reason for restoration of edentulous space.

Effect of birth rank on the caries experience of children from a suburban population in Nigeria

Abstract
The study aims to explore the possible impact of biological factors such as birth rank, age and sex on the severity of caries experienced by a population of Nigerian children with caries. Specifically, it explores the impact of a child’s birth rank on the severity of caries and the effect of other biological variables such as the age and sex on this risk. Information were collected from the 497 children from 135 families. Information collected included the age of the child, the gender and the birth rank. The age of the child was the age at the last birthday. The dmft/DMFT was also assessed for each child recruited as well as that of his or her siblings. The model uses the first child as the reference. It compares the first and the second child and so on. The trend of association was also tested. The strong risk factors identified for caries in this study are the age, having older siblings with caries and sex. The dmft/DMFT reduced by 0.072 for every year increase in age. Also, the dmft/DMFT is less by 0.25 in females compared with males. Finally, the dmft/DMFT of a child increases by 0.48 for every older sibling having caries. While there was an associated established between birth rank and caries, only age, sex and having older siblings with caries were established as risk factors for caries in the children in this study population. The finding of this study helps to provide clinicians an additional evaluation tool for identifying caries risk of children seen in the dental clinic. Patients with older siblings who have caries should be managed as a high caries prone child.
