A modified technique that decreases the height of the upper lip in the treatment of gummy smile patients: A case series study

Abstract

Delayed passive eruption is conventionally treated with osteotomy and osteoplasty, which is limited to the vicinity of cementoenamel junction. The aim of the present study was to evaluate clinical outcome of three gummy smile patients with delayed passive eruption submitted to an extended osteoplasty in the buccal aspect of anterior maxilla. Digital photograph images of maximal smile were taken in order to assess the results before and 9 months after surgery. All maxillary incisors were evaluated. In comparison with baseline, all patients presented lip dropping (calculated in 7.9 ± 1.7%), increase in crown length (18.2 ± 2.7%), and a decrease in the gingival display (46.6 ± 2.7%). On the other hand, considering specifically the decrease in gingival display, 19.7 ± 3.8% of it was due to the amount of lip dropping, while 26.9 ± 1.2% was due to the increase in crown length. Within the limits of the present study, it could be concluded that after the extended osteoplasty, upper lip has slightly dropped and was less tensioned, which provided a more harmonious smile.

Abstract

Distraction osteogenesis (DO) techniques have been widely accepted and practiced in orthopaedics, traumatology, and craniofacial surgery. Over the last two decades, using DO methods, many previously untreatable conditions have been successfully managed with outstanding clinical outcome. Although the mechanism of DO is still not fully understood, it is generally accepted that mechanical stimulation is the key in promoting and maintaining tissues’ regenerating capacities. In the management of severe maxillomandibular deformities, orthognathic surgery produces less than optimum outcome with greater chances of relapse. Inadequate bone contact, insufficient fixation, stability and partial or total relapse due to excessive muscle stretching are often observed. To overcome these disadvantages a path breaking work in the field of distraction, osteogenesis occurred. The present article aims at highlighting the advantages of this newly developed technique over previous surgical remedies.

Pemphigus vulgaris associated with significant periodontal findings: A case report

Abstract

The purpose of this article is to report a case of Pemphigus vulgaris (PV) associated with periodontitis and role of dermatologist and periodontist in its management in the routine practice. Periodontitis is a plaque induced inflammation involving and destroying the supporting alveolar bone, cementum and periodontal ligament. Pemphigus is a group of bullous diseases that affect the oral mucosa and the skin, leading to acantholysis that causes painful oral ulceration, making ineffective oral hygiene which allows for the accumulation of more plaque, a causative factor for the periodontitis. A 47 year-old male, a known case of PV presented with a complaint of oral ulceration and burning sensation of mouth and ulcerative skin lesions over the skin, since 3 years. A clinical examination revealed the existence of poor oral hygiene status, bleeding on probing, significant clinical attachment loss and probing pocket depth. Radiograph showed combination of horizontal and vertical alveolar bone loss. This case reaffirms the fact plaque control is the most important procedure in preventing periodontal infection in PV patients. These patients should be informed about the risk of periodontitis, and encouraged to pursue long- term periodontal follow up by the dental professionals to prevent their periodontal disease progression.

IN VITRO  remineralization of human and bovine white-spot enamel lesions by NaF dentifrices: A pilot study

Abstract

The aim of this feasibility study was to evaluate the IN VITRO remineralization effects of four dentifrice systems using microhardness and fluoride uptake analyses. IN VITRO testing for the potential remineralization of the white-spot lesions in bovine and human enamel was performed using a 10-day pH cycling model. The study involved the following NaF silica-based dentifrices: 1) placebo (0 ppm F), 2)  500 ppm F, 3) 1150 ppm F, and 4) 500 ppm F plus functionalized tricalcium phosphate (fTCP). Each day consisted of four two-minute treatments, one four-hour acid challenge (pH = 5.0), and immersion in artificial saliva (pH = 7.0) between these events. After cycling, specimens were analyzed for surface microhardness (SMH), enamel fluoride uptake (EFU), and cross-sectional microhardness (CSM). Statistical analyses revealed significant differences (ANOVA, LSD, p<0.05) among the four groups, with the placebo and 500 ppm F dentifrices providing significantly less remineralization relative to the 1150 ppm F and 500 ppm F plus fTCP dentifrices. Notably, while CSM measurements for both enamel types generated similar profiles for the four groups, SMH and EFU revealed human enamel was more sensitive to the 500 ppm F dentifrice groups compared to bovine enamel. This apparent sensitivity may be due to the inherent structural differences between the two substrates.

A novel presentation of a molariform supplemental tooth with dens evaginatus concrescent with a maxillary premolar

Abstract

Concrescence can occur between two adjacent normal teeth or it can also happen between a normal and a supernumerary tooth. Dens evaginatus are uncommon and usually arise from the occlusal surfaces of mandibular premolars or the palatal surfaces of the maxillary incisors. Evaginatus odontomes have rarely been seen associated with supernumerary teeth. We describe a very rare combination of supplemental tooth with dens evaginatus concrescent with a maxillary first premolar in an otherwise healthy 13 years old boy with a chief complaint of discomfort in maxillary left quadrant. Recognition of these conditions and early diagnosis are important to avoid complications.

Oral status of a group of cerebral palsy children

Abstract

The purpose of the present study was to evaluate the oral health status (dental caries and dental bacterial plaque), malocclusion, brushing frequency and drooling, in accordance with the type of cerebral palsy in a group of children with cerebral palsy. A total of 25 children with cerebral palsy between the age range of 6.17 to 10.5 years were examined. 25 controls were chosen, that were age and gender matched. The subjects had either spastic or mixed type of vertebral palsy with an average calculated deft and DMFT of 3.0. Class II malocclusion was most prevalent in the cerebral palsied children. The problems of cerebral palsied children must be carefully understood and treated, in order to accomplish desired results in terms of complete dental care of the highest quality.
