Study of premenstrual syndrome (PMS) on quality of life at a University Hospital of Sindh

Abstract

The objective of this study is to observe the magnitude and impact of premenstrual syndrome on quality of life. Prospective, observational study was used in conducting the study. This study was carried out for the period of one year that is, from 30th June 2010 to 31st June 2011. There were 280 females age between 15 to 49 years, selected by convenient sampling who visited the Gyne/medicine OPD Liaquat University Hospital Hyderabad, during study period. The female, irrespective of parity with symptoms suggestive of premenstrual syndrome were included and those who were showing irregular menstrual cycle, other gynaecological and medical disorders such as thyroid disorders were excluded. Data was collected by filling a 29 variables shortened premenstrual Assessment form based on Moos Menstrual Distress Questionnaire after taking consent from the study participants. The severities of symptoms were assessed on both criteria. That is, ICD-10 (Tenth Revision of the international classification of disease) DSM-IV (Research diagnostic criteria for PMDD). A total of 280 women of young age were taken to see the frequency of premenstrual syndrome. The Mean ± SD of women age was 27.5 ± 4.22 (15 to 49 years). The frequency of Premenstrual syndrome (PMS) was more in women between 25 to 35 years. The less frequency was observed on women of age between 14 to 24 years. The clinical presentation like irritability was seen in 63.6%, abdominal cramps 47.9%, moderate to severe headache 41.5%, decreased performance of work at college was seen in 30.7%. The frequency of anxiety was 25.4%, crying in 23.9%. The depression was commonly seen in 71.1%. Premenstrual syndrome (PMS) is still a common problem and should be diagnosed on standard criteria, proper treatment enhances the quality of life.

Hematological and temperature profile in micronutrient based combination therapy of uncomplicated falciparum malaria in under five’s

Abstract

This study was conducted to elucidate the influence of some selected antioxidant micronutrient combination on hematological parameters in the course and therapeutics of malaria in early childhood. 150 participants (6 months to 5 years) diagnosed with uncomplicated falciparum malaria were recruited for the study from two Health Centres in Ekpoma, Edo State Nigeria, after obtaining ethical clearance from the ethical review Board of the Edo State Ministry of Health. The participants were randomized into 15 cohorts of 10 patients each. Additionally, 20 apparently healthy volunteers were used as control for the hematological parameters assessed. The patients in the active comparator group were administered with oral doses of standard artemisinin based combination therapy, while the interventional cohorts were administered varying combinations of antimalarials (Artesunate or Amodiaquine) and micronutrients (vitamin A, E, zinc and selenium). A comparative analysis between the total white blood cell (WBC) count on day 0 and day 28 post treatment revealed a significant increase in all the micronutrient treated groups on day 28 (p<0.05; r = 0.76; P<0.01). In addition, there was a negative correlation between the parasite density, packed cell volume (PCV) and hemoglobin concentration in the study population (r = -0.102; P >0.01; r = -0.08; P>0.01 respectively). Conclusively, the use of varying bi-combination of antioxidant micronutrients as adjuvants to standard antimalarial agents has immunomodulating potential that may be of benefit in malaria therapeutics.

Giant solitary colonic diverticulum presenting as an abdominal mass: A case report

Abstract

Diverticular disease of the colon is a common disease in the Western population. Giant colonic diverticulum is a rare presentation of this disease. We report a case of a 38-year-old female suffering from chronic kidney disease, who presented with chronic constipation and meteorism progressively getting worse over a number of months. Pre-operative imaging revealed a solitary sigmoid mass which later turned out to be a giant diverticulum which was compressing her bladder and ureters. Bilateral ureteric stents were inserted and an anterior resection performed. Recovery of patient post-operatively was uneventful. Histopathology report confirmed a perforated inflamed giant diverticulum in the sigmoid colon. Pathophysiology of giant colonic diverticulum is discussed.

Assessing the awareness and acceptance of recent guidelines for swine flu before and after education programme

Abstract

H1N1 influenza is a respiratory disease of pigs caused by type A influenza virus. The aim of this study is to evaluate awareness and attitude towards swine flu amongst the people. Data were collected in form of structured closed and open questionnaires in face to face interview in a pre-coated and easy to analyze format with or without probing elicit specific response. The persons of either sex, attended programme at K.I.M.S. Karad for swine flu examination were enrolled. Among participants 60% were male and 40% were females with more than 90% having education above primary schooling level. To all 250 persons about 75 questionnaires were distributed before awareness programme. After one week of programme the same questionnaires were collected from them, those who attended the programme. The comparative status showed after the educational programme, acquired more information about the prevention and treatment of swine flu. The knowledge of swine flu is very poor among the public. The fear of pandemic was not able to crush peoples’ spirit for leading a good life and most of patients were ready to help prevent its spread.

Serum cardiac troponin T (cTnT) in Nigerian children with sickle cell anaemia: an index of myocardial injury?

Abstract

Recurrent vaso-occlusion in sickle cell anaemia (SCA) may be associated with myocardial ischaemia. We determined the serum cardiac Troponin T (cTnT) profile in children with SCA and assessed its relationship with the vaso-occlusive crises pain severity scores. Serum Troponin levels of 26 children with SCA who had significant painful crisis  (cases) and 27 SCA in stable state (controls) were determined using microparticle enzyme immunoassay method. The severity of the pain among the cases was determined using age appropriate pain rating scales. The overall mean (Standard deviations, SD) serum cTnT for the 53 children was 65.63(23.54) ng/L with 45.3% having normal serum levels. Cases however had significantly higher mean serum cTnT than the controls, 119(24.40) vs. 57.90(13.80) ng/L, p=0.001. Also, significantly higher proportion of the cases, 19 (73.1%), than the controls, 3 (11.1%) had elevated serum cTnT levels (i.e. serum levels >100 ng/ L), p=0.001. Serum cTnT levels had strong positive correlation with the pain severity scores; Pearson’s correlation coefficient (r) =0.64, p=0.011. Routine serum cTnT estimation may be useful in evaluating for subclinical myocardial ischemic damage which may be associated with vaso-occlusive crisis in children with  SCA.

Palestinian mothers' and pre-school children’s mental health problems

Abstract

The aim of this study is to investigate the relationship between mothers’ and their pre-school children’s mental health problems. A sample of 324 pre-school children aged 4 to 6 years were selected from 20 kindergartens in the Gaza Strip. Mothers completed the Hopkins Symptoms Checklist and the Children Behavior Checklist. The most common mental health problems among mothers were: headaches (32.4%), crying easily (31.5%), feeling nervous (25.9%), and tension and excitement (25.9%). Of those, 41% of mothers reported severe symptoms, and these were higher if fathers were unemployed.  There was significant association between boys’ overall mental health (emotional and behavioral) problems and maternal depression. Among girls, there was a significant association between their overall mental health problems and their mothers’ total mental health, anxiety, and depression problems. Maternal and child mental health problems are strongly inter-related from an early age. Therefore, universal and targeted interventions should involve both parents and children; enhance positive aspects of their relationship; and be integrated with other health, social care and educational initiatives.
