An investigation of extended spectrum-β-lactamase producing Escherichia coli and Klebsiella pneumoniae causing urinary tract infection

Abstract

Extended spectrum β-lactamase (ESBL) producing Escherichia coli and Klebsiella pneumoniae causing urinary tract infection (UTI) constitutes a significant therapeutic challenge. The aim of this study was to investigate the frequency of ESBL producing Escherichia coli and Klebsiella pneumoniae isolated from urine samples. Clinical isolates of multidrug resistant Gram negative rods (n=102) were taken from urine specimens from tertiary care hospital Lahore. Micro-organisms were identified on the basis of their morphology, cultural characteristics, and confirmed using API 20E. Identified E. coli and K. pneumoniae were subjected to susceptibility testing using Kirby-Bauer disc diffusion method as per CLSI guidelines. Furthermore, phenotypic confirmation of ESBL production was done according to CLSI guidelines. Out of 102 multidrug resistant Gram negative rods 97 (95%) were ESBL producers isolated from different urine specimens from different wards of hospitals (emergency, ICU, urology ward); among these 64 (66%) were E. coli and 33 (34%) were K. pneumoniae. Antimicrobial susceptibility pattern of ESBL producing pathogens showed that 75% of these pathogens were resistant to ciprofloxacin whereas most effective drug was meropenem and pipracillin+tazobactam. Meropenem could be used as an alternative treatment option for UTI related to ESBL producing E. coli and K. pneumoniae. High prevalence of these ESBL producing pathogens are alarming and need special consideration.

The burden of mental-physical fatigue and its correlates in patients undergoing hemodialysis and peritoneal dialysis

Abstract

Fatigue is a debilitating symptom in patients with end-stage renal disease. This study was aimed to determine the level and correlates of physical and also mental fatigue symptoms in addition to its effect on health related quality of life (HRQoL) in hemodialysis (HD) and peritoneal dialysis (PD) patients. A questionnaire-based cross-sectional study involving 318 patients was conducted in three dialysis centers. Data were collected using the Fatigue Severity Scale (FSS), Short-Form 36, Beck Depression Inventory (BDI) and Arizona Sexual Experiences Scale (ASEX). Patient groups were identified as having fatigue (FSS >4.0), depressive (BDI ≥17) and having sexual dysfunction (SD) (ASEX ≥12). Fatigue prevalence was 71.6% in HD patients and 66.9% in PD patients. Depression prevalence was 43.7% and most of the depressive patients (87.8%) had fatigue. SD prevalence was 77.8% and 75.2% of them had fatigue. MCS and PCS scores were markedly lower in fatigued patients. FSS was correlated with BDI, PCS and MCS scores in both HD and PD patients. Fatigue was similarly frequent and severe in both types of dialysis groups and this seriously impaired HRQoL. Depression and physical HRQoL were the major predictors of fatigue in both groups.

A study of Demodex spp. in perinea regions and faces of young men visiting the urology policlinic

Abstract

It has been reported that types of Demodex can be transmitted to other people through close contact and they can play a pathogenic role in rosacea, acne vulgaris, perioral dermatitis, seborrhoeic dermatitis, micropapillary-pruritic dermatitis, and blepharitis. Types of Demodex are reported to be located in various places of human body. The purpose of the present study was to detect the presence of Demodex spp. in the samples taken from the faces and perinea areas of the young men using standardized surface skin biopsy (SSSB). Accordingly, samples were obtained using SSSB method from the faces and perinea areas of 200 men aged 19 - 34, which were then covered with entellan and sent to the parasitological laboratory. Demodex spp. was found in 42.0% of the samples taken from perinea area and in 83.0% of the samples taken from subjects’ faces, while Enterobius vermicularis was detected in two cases. One of the samples taken from was Demodex brevis, while the others were Demodex folliculorum. It was concluded that, given that species of Demodex can be found in perinea area under conditions of collective accommodation, specimens should be taken from patients consulting with pruritus or allergic reactions in order to diagnose the parasite, and treatment protocol should be planned according to the results of the analysis.

A study of synergistic therapy of enalapril and Cordyceps sinensis in the development of renal function in chronic allograft nephropathy patient

Abstract

Chronic allograft nephropathy (CAN) still remains an important factor that affects the long-term survival of renal recipients. The aim of the study was to investigate synergistic effect of enalapril (an angiotensin converting enzyme inhibitor, ACEI) and Cordyceps sinensis (Bailing capsule, fermented agent of C. sinensis) on CAN and pursue an effective therapy to control CAN progression. A total of 84 CAN patients who underwent transplantation (live related donor, no prisoners were used in this study) were involved in the study and randomized into four groups. Group A (n=22) received combined treatment of enalapril (10 mg/day) and C. sinensis (2.0 g/per times, three times per day), group B (n=20) was treated with enalapril (10 mg/day), group C (n=21) with C. sinensis (same dose as in group A) and group D (n=21) treated with immunosuppressive agents was set as control. Serum creatinine (SCr), blood urea nitrogen (BUN), creatinine clearance rate (CCr), urinary protein in 24 h (24 h Upro) and urinary transforming growth factor beta 1 (TGF-ß1) of all patients were measured before treatment, and at six months after treatment. After treatment for six months, SCr and CCr were improved while 24 h Upro and urinary TGF-ß1 decreased in group A, and SCr improved and 24 h Upro decreased in group C. Patients of group A obtained the highest degree of improvement, and more patients obtained renal improvement and stability than in the other groups. The results of the study show that combined use of enalapril and C. sinensis takes advantages of reducing excretion of urinary protein, improving renal function and retarding CAN progression for CAN patients compared with single use of enalapril or C. sinensis.

Adherence to clinical practice guidelines for anemia among Mexican chronic dialysis patients

Abstract

We analyzed adherence to Clinical Practice Guidelines (CPGs) for anemia in dialysis patients, and the frequency with which the target ranges for treatment parameters were reached. A questionnaire to analyze retrospectively the adherence to CPGs for anemia was performed. Patient’s files were reviewed and extracted demographic, clinical, laboratory and treatment data. In a cohort prospective study, we made clinical measures of hemoglobin (Hb), hematocrit (Htc), iron (Fe), transferrin (TF) and transferrin saturation (TSAT). Adherence was based on frequency of biochemical evaluations and the target ranges for the variables studies in the 2008 version of the KDOQI guidelines for anemia management. 573 patients were studied, 25% of the clinical files lacked recording of Hb and Htc. Less than 10% of the patients had one or more reports of ferritin or serum iron and none had reports of TF or TSAT. Only 40% of patients were within the limits of Hb suggested by CPGs. Over 60% of the patients had TSAT normal. Our data showed low degree of adherence to CPGs for anemia. The frequency with which the target ranges were reached was far of theoretical objectives.

Analysis of proteomic and acute kidney allograft rejection in rat

Abstract

Kidney transplantation to treat end-stage renal disease has evolved rapidly from the first successful transplantations to the current widespread use of grafts from both cadaveric and living donors. But acute rejection is still a strong risk factor for chronic rejection in recipients of renal grafts. To investigate possible mechanisms, we describe a comparison between different proteins expression profile of acute rejection and the controls. Through two-dimensional difference gel electrophoresis, mass spectral techniques microarray analysis and quantitative real-time RT-PCR confirmation, marked change in expression level of 37 protein spots was observed in allograft rejection group. Among them, expression levels of 24 protein spots were up-regulated, while expression levels of 13 protein spots were down-regulated. Among 37 protein spots, 29 obtained satisfactory peptide mass fingerprinting. We identified 6 proteins in acute rejection after renal transplantation. Then, we investigated PDIA3 levels in serum and transplanted kidney, PDIA3 mRNA and protein expression level. Our data thus indicate that PDIA3 might be potentially involved into the occurrence and development of acute rejection response in renal transplantation.
