An unusual cause of acute tympanic membrane perforation: A case report

Abstract

This is a case report of a 26 – year old lady who presented in the Accident and Emergency Department of our hospital with complaints of sudden onset of right otalgia, tinnitus and loss of hearing which she claimed started at about 12:30 h after vigorous kissing incident. The tympanic membrane was ruptured. She was managed conservatively with oral antibiotics with spontaneous and complete healing of the perforated tympanic membrane within one month of follow-up. It was shown that ear kissing can lead to auditory injury and loss of hearing and so kissing should be made as gentle as possible in all circumstances.

Could we reduce the incidence of methicillin-resistant Staphylococcus aureus (MRSA) infection in Otitis externa

Abstract

The incidence of MRSA is once again on the agenda for the hospital management. This strain of micro organism presents a challenge for ENT doctors worldwide. Despite many strict measures taken all over the world, the figures appear to be rising; there has been a steady increase in the number of cases of nosocomial MRSA infections. We examined all external ear swabs samples taken from patients complaining of Otitis externa clinically, in the Royal Victoria Eye and Ear Hospital from 1st January, 2004 to 12th December, 2004. We analysed the results of the compared MRSA O. externa to previous study conducted 8 years ago in the same hospital.

Epidermal cyst of the bony external auditory canal in adult

Abstract 

The objective of the study is to present a rare case of epidermal cyst of the bony external auditory canal in adult. Epidermal cyst of the bony external auditory canal, though very rare, was kept in the list of differential diagnosis of a skin lined mass of the external auditory canal. A case report method and review of literature was used for the diagnosis. Epidermal cyst is very rare in the external auditory canal. Only two cases of epidermoid cyst arising from the bony external auditory canal are reported previously in English. But both were in pediatric age group. Epidermal cyst in external auditory canal in adult patients may be confused with masses that are commonly seen; this includes osteomas, exostosis, ear polyps, carcinomas etc. Epidermal cyst should be included in the differential diagnosis of a patient with an ear mass.

Antibiotics resistance and susceptibility pattern of a strain of Staphylococus aureus associated with acne

Abstract

Staphylococcus aureus was isolated from an individual with acne. The isolate was Gram positive, spherical and in clusters, golden yellow on mannitol salt agar and non spore forming. Biochemical tests showed that it was unable to hydrolyze gelatin but able to ferment glucose, galactose and mannitol. It produced catalase and coagulase enzymes. It was able to utilize citrate as sole carbon source but was indole negative. It was Methyl Red positive and Voges-Proskauer negative. It was sensitive to gentamicin, tetracycline, amoxicillin, augmentin, chloramphenicol and sulphamethoxazole but resistant to ampicillin, erythromycin, cloxacillin, cotrimoxazole, streptomycin and penicillin.

Risk factors for ventilation associated-pneumonia by multidrug-resistant Pseudomonas aeruginosa producing metallo- -lactamase

Abstract

To evaluate if risk factors for ventilation associated-pneumonia (VAP) by Pseudomonas aeruginosa differed among patients affected by P. aeruginosa producing metallo- -lactamase (MBL-PA) or non-MBL-PA isolates. A 1-year retrospective case-control study was conducted in a surgical-clinical intensive care unit at a tertiary-care university hospital. Cases were patients with MBL-PA VAP and controls were those with non-MBL-PA VAP. Univariate and multivariate analysis were performed to identify risk factors. A total of 40 cases and 53 controls were included in the study. Univariate analysis showed that a high-risk ASIS score, clinical category in hospital admission, prior hospitalization, length of ICU stay, length of mechanical ventilation, exposure to three or more antibiotics, exposure to carbapenems and fluoroquinolones were risk factors for VAP by MBL-PA. After logistic regression, the prior hospitalization and length of mechanical ventilation continued significant. The mortality was also independently associated with MBL -PA VAP. Hospitalization within the preceding year and length of mechanical ventilation were risk factors for MBL-PA VAP, deserving attention and further investigations. MBL-PA VAP resulted in higher mortality rates most likely related to the commitment effective treatments of nosocomial infections by this organism. Early identification of patients with risk factors which could guide this initial appropriate empirical therapy and the development of strategies to prevent the dissemination of MBL-PA strains.

Experimental chronic inflammation induced in mice periodontium by Actinomyces israelii entrapped in alginate gel

Abstract

Experimental chronic inflammation in periodontal actinomycosis was studied with INCQS standard Actinomyces israelii infected mandibula in Swiss/NIH mice (n = 45), male and female with 21 days of age. Tissue responses of the experimentally induced chronic inflammation were investigated in mice by clinical and light microscopy. After anesthetization, the animals were inoculated in paraperiosteal periodontium anterior mandibula with 108 UFC/mL A. israelii (n = 45) associated with sodium alginate gel particles. The animals had been evaluated clinically and microscopically at 1, 3, 7, 15, 21, 30 and 45 days after inoculation. Chronic inflammation was induced in all the mice when compared to the control animals. As such, both groups had normal general health throughout the study. In the control mice (n = 10), no lesions were noted. However, the clinical and histopathology evolutions of actinomycosis lesions had been observed and are discussed in this work.
