A study of the prevalence of potentially inappropriate medications (PIMs) in Thai adults

Abstract
Potentially inappropriate medications (PIMs) are a common problem in older adults and are associated with negative outcomes. The objectives of this study were to evaluate the prevalence of and the factors associated with the use of PIMs by elderly patients in an outpatient setting of the tertiary care hospital. A retrospective medical record audit was randomly reviewed in 308 elderly patients in 2010. Beers criteria (2003) and the screening tool of older persons’ potentially inappropriate prescriptions (STOPP) were used to identify PIMs. The results showed that the median number of medicines per patient was 5.6 (inter-quartile ranges 3.5 to 7). Prevalence of PIMs determined by Beers criteria and STOPP was 19.2 and 31.5%. The only factor that exhibited associations was the higher number of prescription medications based on STOPP criteria (odds ratio 1.2, 95% confidence interval 1.1 to 1.4), p<0.05. PIMs are highly prevalent among older adults in the tertiary care setting and are associated with greater number of medications based on STOPP. Beers criteria are a less sensitive tool than STOPP to detect PIMs for Thai older adults.

A study of an ideal animal model for evaluating the potential allergenicity of novel foods proteins

Abstract
Food allergy is now an important health issue, and there is urgent need for a developmental approach to identify allergenic potential of food. We present an approach that shows some promise for assessment of shrimp allergenicity using BALB/c strain mice. The mice were immunized by intraperitoneal injection of shrimp allergen. Mast cell degranulation in combination with serologic methods was used to monitor protein allergy. The results showed the method could continuously reflect the variation of allergic symptom and actualize dynamic determination of shrimp allergenicity. Furthermore, it is feasible, sensitive and repeatable. The approach will provide some valuable reference for identifying allergenicity of novel food proteins by using animal model in the future.

Prevalence of anemia in the diabetic elderly: Clinical correlation and comorbidities

Abstract
This was a prospective, cross-sectional study with descriptive and analytical purpose that was carried out over a twelve-month period. A sample of 181 diabetic elderly was selected. Data were collected in the Department of Internal Medicine and Geriatrics at the Treichville University Hospital, using a survey form coupled with standardized geriatric assessment (SGA) cards. The parameters of anemia and diabetes were quantified and assessed. The comparison of the qualitative variables was done using the Chi² test and a P≤0.05 value considered to be significant. The prevalence of anemia is very high in the population of the elderly with diabetes (79.56%). We find a male predominance with a proportion of 60.77%, and a sex ratio of 1.54. The type of anemia was dominated by microcytic hypochromic anemia and macrocytic normochronic anemia, both with the same proportion of 42.36%. Factors in correlation with anemia were renal failure The most common risk factor was hypertension (72.92%) while the specific comorbidities of the elderly were malnutrition (40.88%), depression (72.38%) and cognitive impairment (69.61%).This study allowed us to know the prevalence of anemia, the correlations and the comorbidities commonly observed in the diabetic elderly.

The acute renal insufficiency in medical reanimation at Lomé

Abstract
The objective of this study was to investigate the profile of acute renal insufficiency in medical reanimation. It was a retrospective and analytical study which took place from 1 January 2011 to 31 December 2012 at the medical recovery room of the Sylvanus Olympio Teaching Hospital in Lomé. All patients with a minimum of a plasma creatinine ≥ 150 umol / l and / or blood urea ≥ 10 mmol / l were included in this study. During our period of study, 1295 patients were admitted into the medical reanimation of the Sylvanus Olympio Teaching Hospital (CHU) in Lomé and 53 patients had met our inclusion criteria and were the subject of this study. Our patients were most admitted for dyspnea (42.3%), headache (25%). The average creatinine was 138.5mg / l with extremes of 15mg / l and 262mg / l. Fifty patients (94,34%) were suffering from stage I renal pains. The main etiologies of renal insufficiency were HIV (45,2%) malaria (30,2%). ARI are frequent in the medical recovery at the Sylvanus Olympio Teaching Hospital of Lomé. Plasmodium falciparum malaria and HIV are the major causes.

Abstract
We present one observation of the Rosai-Dorfman and Destombes disease (RDDD), particularly under its revealing form and by its favorable evolution under corticosteroid therapy. It was a patient 29 years old, who presented multiple cervical and abdominal adenopathy. All operating in the alteration of the general condition and fever. The diagnosis is based on histological evidences after ganglionic biopsy which showed a sinus lymphohsitiocytosis. The evolution has been favorable under corticoid treatment. The Rosai-Dorfman and Destombes disease is a rare benign affection, thus difficult to make diagnosis.

Abstract
Sclerosing angiomatoid nodular transformation of spleen is a benign primitive vascular tumor of unknown etiology first described in 2004 by Martel et al. This condition can occur at any age with a female predominance but has been rarely described in children. The clinical signs found in our study are mainly splenomegaly and weight loss occurring in a context of chronic inflammatory syndrome and mixed microcytic anemia in a 39-year-old patient. CT and abdominal MRI used preoperatively helped the diagnosis of sclerosing angiomatoid nodular transformation of spleen (SANT) even though the radiological characteristics of splenic lesions observed were little specific. Immunohistochemistry after biopsy of the spleen helped make the diagnosis of certainty of SANT. Pathological examination of SANT reveals a splenic parenchyma replaced by well circumscribed angiomatoid nodules separated by an inflammatory fibro sclerotic stroma. Splenectomy by laparoscopic surgery with curative purpose ended all chronic inflammatory process maintained by this benign vascular lesion of spleen.
